Horse History – admission date, name, breed, date/year foaled, name of farrier and phone, date trimmed/shod, name of vet and phone, date of last shots, list of shots and a copy of the Coggins Report

 

Horse Injury History – if the horse will be coming for rehab please include all information available from the vet – diagnosis and prescribed treatment
